Xpress Account Opening Form Instructions:

You can use this form to open People's United Plus deposit accounts for individual or joint account owners (personal use only). This form cannot be used to open custodial or trust accounts - please visit a People's United branch to open these accounts.

These instructions are to assist you in completing the Xpress Account Opening Form. If more than one name will be on the account, the name entered as the Primary Account Holder will be responsible for tax reporting. If you open more than one account using this form, the same names will be on the accounts.

Step 1

Complete the New Account Form:

Account Type - Please indicate the type of account you are opening.

Be sure to include the amount of your opening deposit. This can be made by check or money order, no cash please. If the deposit amount is being split between two or more accounts, indicate the amount to be allocated to each account. Minimum deposit requirements are indicated on the form.

Customer Information - Please indicate if you are a People's United Bank customer. Complete all requested information for each name on the account.

Personal Identification Number (PIN) (for Checking accounts only) - Choose a 4-digit number which will be used for Telephone, Plus ATM Card and People's United Online (TM) transactions.

Check Ordering Information (Checking customers only) - List the name(s), address and other information you would like to appear on your check order. A Post Office Box cannot be accepted.

Authorized Signature(s) - All account owners must sign and date the form.

Certification of Taxpayer ID Number - Cross out the information in subsection (2) of this section if the Primary Account Holder is subject to backup withholding.

Step 2

Enclose a check or money order payable to People's United Bank. Send one check for the total of all deposits indicated on the new account form. Please do not send cash.

Step 3

Mail the completed form, your deposit and a copy of your bank approved form of identification to People's United Bank, P O Box 7001, Bridgeport, CT 06601-9940, or visit your nearest People's United Bank branch, whichever is more convenient for you. Please use the night deposit box after hours

People's United Bank Approved Forms of Identification

The primary form of identification must be one of the following: Connecticut Driver's License

• U.S. Passport
• U.S. Military Identification Card
• Connecticut Department of Motor Vehicle Identification Card (CT DMV ID)
• Permanent Resident Alien Card

Forms of secondary identification include:

• a current employment identification
• a current utility bill
• a major credit card (e.g., VISA, MasterCard, Discover, American Express)
• recent pay stub
• Medicare card
• current student indentification card
