Please note that all health and human services organizations are funded exclusively through our contribution to the United Way in order to utilize their expertise in distributing the funds. Also, People's United Bank does not generally fund capital campaigns. We prefer to contribute to specific programs and events.
Please provide the following information:
- A cover letter on the requesting organization's official stationery that states the amount requested and the purpose of the grant. Please indicate clearly the name and the telephone number of the contact person.
- A brief description of the organization's purpose, mission, history and accomplishments. Please limit this information to one page with12-point font and one-inch margins.
- A list of government, corporate, and foundation grants, investments or loans, and the most recent amounts from these contributors to the organization or project.
- A letter of tax-exempt charitable status under Section 501(c)(3) from the Internal Revenue Service and proof that the organization is not a private foundation as defined in Section 509(a) of the Internal Revenue Code. If a fiscal agent is to be used, attach the written agreement with the fiscal agent and a copy of its 501(c)(3) status letter.
- Names and affiliations of the Board of Directors and board sub-committees, if appropriate, frequency of meetings, and role in the organization.
- List of accrediting agencies, where appropriate.
- Report on accomplishments from the last grant received from People's United Bank, if appropriate.
- A plan to evaluate a measurable impact of a People's United contribution.
- The organization's operating budget for the current year and its proposed budget for the next year, or the project's operating budget for the current year and its proposed budget, upon request.
- The most recent audited financial statements and the most recent Form 990 (the combined federal and state charitable report) upon request.
